I don't see any loopback devices installed so there's a problem somewhere.

yes, you're right, on my Ultra20, i have some problems:
Oct 4 08:47:01 ultra20 vmix: [ID 968132 kern.warning] WARNING: Attach failed
Oct 4 08:47:06 ultra20 last message repeated 1 time

i have to reboot to see what's happen
